In aviation, small mistakes can have disproportionate consequences. A single-digit error in a Life-Limited Part (LLP) record may seem trivial, but it can have far-reaching effects on safety, operations, and compliance. This case study examines how one extra zero — an easily made clerical error — could have escalated into a serious airworthiness issue, and what lessons it offers for maintenance teams and aviation organizations.
Context: Life-Limited Parts and the Absolute Need for Accuracy
A Life-Limited Part (LLP) is a critical aircraft component that must be permanently removed from service once it reaches its certified life limit, as defined in the Aircraft Maintenance Program (AMP) and/or mandated by an Airworthiness Directive (AD). These mandatory limits, set by the manufacturer and regulated by authorities, ensure the part remains safe and airworthy throughout its operational life.
Defined Limits: LLPs are certified with a specific life limit, which may be expressed in flight hours, flight cycles, or calendar time, depending on the part and manufacturer specifications.
Examples: Critical aircraft components subject to high stress or load, such as engine rotating parts, landing gear components, and structural elements certified with a safe-life limit.
Safety-Critical: Exceeding the limit can cause structural failure or system malfunction, endangering passengers and crew.
Strict Tracking: Operators must record the relevant parameter for each LLP — whether flight hours, flight cycles, or calendar time — as defined in the AMP or applicable AD. These values are entered and maintained in the Maintenance Information System (MIS). The system updates cycles and hours automatically from flight data or manual entries and alerts when parts approach their limits. It is also used during maintenance planning to create work scopes, ensuring parts due for replacement are included.
Data Entry Mistake
A landing gear pin had a revised life limit of 5,000 flight cycles. During an update in the MIS, an airworthiness engineer inadvertently entered 50,000 flight cycles, adding a single zero, and the aircraft continued operating normally. At this point, the risk is latent: if the error would go unnoticed, it could led to severe safety consequences once the part exceeded its true life limit.

In a less positive scenario, the error persists, and the part has already exceeded its certified life without being replaced. The safety risk increases, and the aircraft is no longer airworthy, though no failure has yet occurred. Replacing the LLP at this stage would involve grounding the aircraft, potential downtime, and significant financial costs, including part procurement and possible disruption to flight schedules.
In the worst-case scenario, the part fails while in service, potentially causing a structural failure or landing gear malfunction. Safety consequences could be catastrophic, putting passengers, crew, and the aircraft at serious risk. All triggered by a minor data entry mistake.
How Safety Nets Work

Quality Control: Critical changes to the AMP are generally verified — both from the source documentation to the AMP, and from the AMP to the Maintenance Information System.
Internal Audits: Auditors cross-check source documents against the AMP, and the AMP against LLP status and remaining life. Discrepancies are flagged for corrective action.
Regulatory Oversight (including Airworthiness Review Certificate in EASA and equivalent environments): Airworthiness inspections require complete LLP documentation; deviations are flagged to prevent unsafe operation.
Scheduled Overhauls: For LLPs installed on major components, such as engines and landing gears, overhaul inspections usually respect the certified limits. Even if an error exists, the work scope review can catch implausible values.
System Error-Proofing: Many maintenance systems include automatic plausibility checks, warning if values are outside expected ranges.
Even so, no system is completely immune, especially for short-life LLPs not individually visible outside overhaul or critical inspections.
Built-in Quality

By analyzing the incident through the Dirty Dozen lens, it becomes clear that human performance alone cannot fully explain the event; organizational processes, system design, and team interactions all contributed to the latent risk.
Under the HFACS framework, this event is classified as a skill-based error — an unintentional mistake during routine data entry. The preconditions identified in the Dirty Dozen analysis increased the likelihood of the error. While oversight mechanisms existed, latent organizational and procedural weaknesses meant the error could have gone unnoticed. Preventing similar events requires addressing both human and organizational factors, including process design, verification procedures, and organizational culture, to mitigate latent risks before they manifest.
